Additional StreetLeader Paperwork

StreetLeaders also need to complete the following forms and background checks:

• Work Permit (go to your high school office for this)

• W-4

• I-9

• Request for Criminal Record Check

• Pennsylvania Child Abuse History Clearance

• FBI Federal Criminal History Report & Finger Prints (Employee Code: 1KG756 Volunteer Code: 1KG6ZJ) (Hours: Mon-Wed, 9-4)

  - For students without government issued photo ID, 2 options:
  1. PA Photo ID Waiver (need parent with you)
  2. Apply for a PA ID Card (PennDOT Office open 8:30-4:15 Mon-Sat and until 6 PM on Thursday)

  - Fingerprint payment options: credit or debit card, money order, cashier’s check

• Direct Deposit Form (you must have a bank account to complete this form)

All employees are required to complete these forms and background checks before beginning work with youth.
